Triage (Initial) Appointment

Bluestem Center focuses on understanding and treating behavior and emotional problems in children by looking at the child in the context of their family. We are serious about wanting to work with the family and to form a treatment relationship with the child and family for the long haul. This means that we take enough time and work in enough depth during the evaluation to understand the many aspects of a child's life that are involved in the development of problems. These same areas can be used as assets in developing a treatment plan.

We believe that our evaluation process is uniquely efficient and thorough. The assessment is organized into seven Domains:

  • The Triage appointment includes a 15 minute meeting with Patient Services to review Bluestem Center's Patient Information and Policies prior to meeting with a staff member to address:
    • scheduling future appointments
    • our billing policy
    • fee and payment schedules
    • after hour services
    • completion of required business and HIPAA forms
  • You will be asked to complete questionnaires about your symptoms
  • Please bring your insurance card to the first appointment
  • If you have worked with other health care providers in the past, you may be asked to sign a release of information form for each
  • If the patient is a minor child, both parents should attend the Triage appointment without their child
  • If special accommodations are required, please inform the receptionist prior to the appointment
